Because 40 year old bolts and/or welds busting loose couldn't possibly create puffs of rust, dirt and debris. There were no "little explosions", there were "violent failures". Supposedly, pilings became more common place after the Florida Sunshine Skyway Bridge disaster in 1980...the Baltimore bridge opened 3 years prior. Plus at that time, cargo ships were much smaller so I assume the designers assumed it was low risk.
