It's midnight & I'm hanging out at the hospital waiting for surgery #3 of the evening to start.

I've been busy w/ the new office so I haven't posted as much lately.

2 large infections & 1 broken jaw tonight.

Random question, how many people extract wisdom teeth under a local vs being knocked out? My wife looked at me like I have two heads when I told her I was awake and drove home after. Uppers were easy, lowers were badly impacted and came out in pieces.

In general oral surgeons pretty much knock everyone out (deep sedation or general anesthesia). Most general dentists do it awake because they don't have the permits & training to knock people out.

I have removed plenty with local only but it takes a special person to tolerate that well. Most people are nervous wrecks if they have it down under local only.

In my practice the fee is the same for local or sedation. Also, I always tell people no one has ever woke up & told me they wish they had stayed awake. Definitely happens the other way, though.

Back in the 60's I was in the USAF and stationed on a Navy Ammunition depot. USN and USAF didn't get along very well. The medical facilities were all Navy. I had 3 wisdom teeth extracted by a Navy dentist without any anesthetic. Not much fun but I didn't make a sound.

Random Google image & a slightly different technique but you get the idea.

The broken fragment gets pulled deep into the infratempral fossae. You fish it out being careful of the adjacent jugular vein then use plates and/or screws to hold it in place.

I've had my last one done on a local, the only thing that hurt was the bill.

The USAF tried for 24 years to remove my wisdom teeth.

Not once did I consent. Not once was my mobility status jeopardized. I still have all four wisdom teeth.

Being in the dental field I have seen people try to go the cheap route. I've worked for plenty of shady dentist here in san antonio. Yeah, you can always find someone to do it cheaper. Will you get the results you want, maybe. Will that dds be there if things go wrong? Probably not. It cost a boatload of money to have a dentist fix another's mistakes.

USN did all 6 of mine. One of them they had to cut in two and pry out with about a 3 foot crowbar.
    That didn't feel great.

I felt no pain, even when he broke the lowers to extract. It's pretty darned loud in your head when they pop though. I'm not a nervous sort thankfully, but I'm guessing the reason sedation wasn't offered is like you said - probably didn't have the license. This was done when I was young and was going to a dental clinic for work. My girlfriend was a dental assistant in the ortho section though, so I felt comfortable with everyone.
