Now days at most places it's computerized and the forms come from a software company and the questions you are asked depend on the company whose software they bought and the staff is often audited on how completely they fill out the forms. Vaccination status is a legit question about your medical history.Some years ago, I had to go to the Urgent Care center for a pinched nerve in my back. The nurse was taking my information as she was checking my vitals. She asked me about my guns, how many I owned, if I ever felt like hurting myself or others. For about a minuted or so, I was caught off guard by her questions. I thought for a second, then asked her how much money did she have in her checking account, and how was her sex life? She looked at me with lots of anger in her eyes! Told me that was NONE of my business! I told her IF I owned guns, or how many, wasn't any of her business either! She informed me she was required to ask these questions.
I got up and left and went to the ER across the street. Funny thing, they never asked me those questions. And when I was talking to the nurses and the Dr., they just about fell out laughing!

Not everyone has your attitude. As a nurse I was blamed, daily, for things I had no control over and developed a pretty thick skin and towards the end it got to where I didn't care if people wouldn't answer questions that might impact on their care. I'd just enter "Refused to answer". BTW, our forms didn't have any questions that weren't health related.I don’t blame the nurse, but I also skip questions not directly related to my visit on a particular day. When we were at Ft Hood, my wife’s job was to do data collection and analysis on the expanded questions. Her position was funded specifically to capture data that proved certain PTSD policies were successful.
The data showed certain perceptions about PTSD were flat out wrong and that those wanting help were getting it. She decided not to report on the the questions designed to identify someone that may have PTSD but wasn’t seeking help because the policy leaned towards punitive instead of helpful in those cases.

I once went for testing in a teaching environment where I knew the history taken would be extensive and much of it not related to my condition. I was OK with that, under the circumstances.Medical forms are a joke. My pen does crazy things when I fill them out

But those questions had nothing to do with my reasons for seeking medical attention. I had a pinched nerve in my neck and couldn't even move my right arm or hand. I wasn't depressed or angry or having emotional or mental issues.
Plus not fifteen minutes later, drove across the street to the ER at the hospital, and they never asked those questions. Matter of fact, they thought it rather ridiculous.
Actually, most of the people with chronic back pain have a mental disorder. Depression and chronic pain go hand in hand. That being said, I don’t recall ever asking about firearm possession.
And when I see someone I usually need a review of systems of 10 for 99220 (for admission under observation status for intractable back pain). I think its important to discuss tobacco. I’ll counsel you to quit, discuss various risks and benefits for treatment for a about three minutes and bill a 99406. Oh, by the way, your 50, you should get your colonoscopy soon. I’d be happy to put a referral in. Oh, you have 30 pack years of tobacco, you need a low dose screening CT scan for lung cancer.

I know exactly why I have chronic back pain and issues and it has not one thing to do with any mental disorders! And I'm not in the least bit depressed either!
I'm an adult. I don't want, or need your counseling. Either fix the issue I came in for, or get a real doctor I can see.
I can see now why so many people die from shoddy medical care! You're too busy asking questions and trying to counsel people!
